Rapha Health Care Services has a mission to provide specialized and flexible environment that would assist individuals with acquired brain injuries, mental illness and other disabilities to achieve their optimal level of independence. Our goal is to empower our clients with a feeling of self-control and focus their purpose toward a satisfying life after a brain injury or other disabilities and we believe that quality of life, respect, freedom and dignity are critical areas of consideration in achieving this goal for each client we served. Our customized living/Assisted living homes are located in the nice, safe and quiet neighborhood in Metro areas of MN.
For our home care clients, feel better in the comfort of your own home. We specialize in care and daily living assistance to an array of individuals. Whether you need daily or weekly assistance due to aging, illness, recovery, or rehabilitation, our care givers will provide an individualized service that you can trust

Q: Who lives in your home?
A: Any individual age 18 years and older with some physical or mental health disabilities, who need help to manage their medications, meal preparation, housekeeping, non-medical transportation, general help with ADL etc
Q: How many people live in a particular home
A: 3-4 people in their private rooms, shared living areas, kitchen and 1 bathroom per 2 clients.
Q: What is your staffing ratio to clients
A: 2 Staffs per 4 clients, depending on the needs of the clients.
Q: What type of funding/insurance do you accept?
A: MA, CADI, EW, BI and Private Insurance
Q: What is included in the rent?
A: rent, utilities such as gas, water, sewage, electricity, cable in living area, phone, breakfast, lunch, diner and 2 snacks
